Notes
10 Jul - Leeds named a completely different outfield selection for the second-half in a 4-3-3 formation and skippered by Sol Bamba.  They also brought four reserves but only keeper Charlie Horton was used so the other subs' numbers are not known.  Harrogate's Nathaniel Pells changed into a no.19 shirt at half-time.
15 Jul - Anomalies on the Leeds side of the teamsheet included Ross Turnbull denoted as no.13 but actually wearing no.1, Byram and Purver swapping shirts from those listed and Morison's late elevation to the starting line-up in place of Chris Wood.  In the 81st minute, Turnbull was subbed after York's goal.  Trialist Adam Drury came on for Leeds, almost two years after his namesake's last appearance for the club.
21 Jul - Many sources credited Stefan Reinartz with the winning goal from a scramble in the six-yard box but Václav Kadlec got a decisive final touch and was awarded the goal on Eintracht Frankfurt's website.  Attendance is approximate, taken from news reports on the crowd trouble after the final whistle.
13 Aug - After extra-time, Doncaster won 4-2 on penalties.
19 Aug - The nominated referee for this fixture was Gavin Ward but the game was actually officiated by the 4th official, Andy Davies.  Unfortunately none of the professional journalists present seemed to notice the change and named the original appointee in their match reports.
22 Aug - Substitutes Álex López (Sheff Wed) and Souleymane Doukara (Leeds) came on in the same break in play but Leeds delayed making what was their final change, while they assessed whether Sol Bamba was fit to continue.
21 Oct - In the 64th minute, Fulham's Ben Pringle was substituted immediately after Chris Wood's equaliser from the penalty spot.  It had been awarded for a handball against Pringle.
29 Oct - Craig Conway's goal after 17 seconds is believed to be the quickest one conceded by Leeds at Elland Road.  Blackburn were down to ten men for nearly seven minutes of playing time while Grant Hadley received stitches to a facial wound following a clash of heads with Luke Murphy.  In the end Rovers opted not to risk their captain and replaced him with ex-Leeds defender Matthew Kilgallon.      
13 Nov - For Wycombe, Matthew Bloomfield took over the captain's armband after coming on as a half-time substitute.
20 Dec - Preston goalie Jordan Pickford was shown the red card in the 31st minute but the game was held up while debutant keeper Matt Hudson was readied to come on as substitute; Hudson finally stepping on the pitch to replace sacrificed midfielder John Welsh just shy of the 33rd minute mark.
29 Dec - Referee Mark Heywood pulled a hamstring and was replaced by 4th official, Richard Clark, for the second-half.  Of the five bookings, only Cyrus Christie saw yellow on Clark's watch.  This game was also notable for Leeds President Massimo Cellino's threat to ban Sky TV cameras from Elland Road in protest at fixture disruption. 
9 Jan - The substitutions shown in the 78th/79th minutes came in the same break in play as Rotherham brought on Matt Derbyshire while Giuseppe Bellusci was being treated for cramp, that ultimately resulted in him being replaced by Sol Bamba.
12 Jan - Souleymane Doukara netted after just 12 seconds, thought to be the the third quickest goal in Leeds' history after Jermaine Wright (10 seconds v Burnley in November 2004) and Mark Viduka (11.1 seconds at Charlton in March 2001).  Like Wright's effort, it only precipitated a 2-1 defeat.
2 Apr - Rotherham's Matt Derbyshire was sent off in the 61st minute for an elbow on Gaetano Berardi but play did not restart until the 64th minute after treatment for the bloodied Leeds defender and a home substitution.  
7 May - Brendan Malone was the nominated referee for this fixture but the game was actually handled by the original 4th official, Andy Haines.
